<Company or Producer name> is hereby granted permission to conduct filming work on the premises of the <_____> Museum, <city>, under the terms and conditions outlined in this agreement, on or between the following dates and times:

Use of this agreement or the footage created under terms of this agreement for any purpose other than that stated above, as well as its transfer or sale to a third party, will be deemed a violation of this agreement.
This agreement includes permission to film objects in the Collection of the <________> Museum and/or objects from the collection of <_____>, exhibited in the <_______> Museum under the separate conditions applying to each.
PAYMENT:
A service fee of $X per day (up to 10 hours, or $X per
hour) will be charged for the following: organization of the requirements,
staff, and timetable for the project; for staff supervision, services,
and overtime; security requirements including guards; conservation
supervision; electricians' supervision and assistance, electricity
& telephone requirements; expenses in connection with moving
of objects, works of art, or other Museum property or furnishings;
use of defined areas for dressing & makeup, eating, set construction,
property storage, etc.; acquisition of authorizations from third
parties; and implementation of this agreement.
The Museum will supply at least one professional staff member, one electrician on call, one maintenance worker, and one Museum guard to supervise and assist the crew during all filming work.
Work hours required by <_____> Museum staff for the organization and administration of the project will be included in the accounting, in addition to on-location filming work. Filming work hours will be counted from the time of arrival of the crew on Museum property until the striking of the set, restoration of the Museum property to its original state, and exit of the crew from Museum property.
Payment in advance is required.
CONDITIONS, REQUIREMENTS, AND ADDITIONAL FEES:
Under no circumstances will the director, actors or crew be allowed
to handle objects or works of art in the Museum's collections.
Use of "objects" in the script/filming will require
the creation of reproductions, at the expense of the filmmaker.
All sites and areas required for filming or behind-the-scenes work will be defined and approved in advance. This includes filming "sets," eating and smoking areas, parking areas, set and property construction and storage areas, and office areas.
Security restrictions and instructions as defined and required by the <_______> Museum, must be fully observed. Crew and equipment movement within or on Museum property will be supervised by Museum security staff. All lighting and other restrictions for conservation reasons, as defined by Museum staff, must be fully observed. (N.B.: Available light only in the <named areas of the Museum>.
All special effects, materials, adhesives and stunts must be approved in advance by both conservation and security staff. Failure to abide by these conditions will result in on-the-spot cessation of filming and cancellation of this agreement, with no refund of payment.
Client must provide certificate of insurance, indicating the Museum and its Board of Directors as additional insured. Client will indemnify the Museum against all claims and expenses arising from the production's infringement of copyright, libel and slander, and invasion of privacy.
Client assumes responsibility for and will compensate the <_____> Museum for any damage to Museum collections or property which may occur as a result of filming work, crew and equipment movement, or the handling of works of art/archaeological artifacts.
The following credit line or lines must appear in the closing credits of the film / in the closing credits of each installment of the production in which the footage produced under this agreement appears:

The director, producer and distributor or broadcast organization will be jointly and separately responsible for all their obligations according to this agreement, and each will also guarantee the obligations of the others.
