It has been a long time and I'm not known for a good memory, but...

it just could be that somehow this is one of the AMOA version Sinistars.
RJ, you'll remember your disappointment that we were unable to add the cool
explosion you had done in time for the show, which was a few months before
the final game was released. I vaguely remember something about using a
dripping blood font, although that one is really hazy, but the lack of the
full set of standard attract mode options fits. Also, I clearly remember
that fixing the Sinistar in the center of the screen so you could watch the
explosion was a late fix when we realized you often couldn't see it when it
blew up. I suppose it might even be some other interim version of the
game. In any case it sounds likely that it is a real copy of a pre-release
version, and hence is also likely to have the coveted "easier" difficulty
settings. But I'm also pretty sure it is NOT the one rumored to be lost in
your garage, as the differences noted sound like they come from a much
earlier version.

One other possible thing to check is the copyright message. Again, I don't
remember for sure, but I believe the show version was copyright 1982,
whereas the release version was copyright 1983, and I think this should
show up somewhere on the screen, unless I'm just remembering the updates we
did on the internal ASCII copyright date.
